Founded by the Augustinian hermits of Sant’Agostino around the 14th century, the church passed in 1877 to the Capuchins who restored and modified the building.
The interior has only a nave with three chapels on each side. In the wooden ancon of the high altar, by Giacinto of the Gussola and Liborio from Mantova (1778 – 80), is the statue of the Madonna Assunta by the Ballantis, called Graziani.
The paintings God the Father and in the first chapel on the left a Saint Francesco with the stigmatas are attributed to Guercino. In the convent too are preserved several paintings including The Wedding of Saint Catherine by Amidano and Adeodato Turchi by Biagio Martini.
